Registration Form

Three Methods to Register:

1.Mail: Click on the Printable Registration Form above. Print the form and fill it out. Mail the form and payment to the address below. We will mail you a receipt.

2.Fax:
Fill out the registration form, including your credit card information, and fax it to (707) 838-1264. We will mail you a receipt. Faxes without credit card information will not be accepted. 

3.Walk-in:Visit us at our office Monday-Friday, 8:30-5:30 PM and register at 9291 Old Redwood Hwy, Building 300D, Windsor

Methods of payment accepted:
-Cash or Money Order
-Check made out to the Town of Windsor
-VISA or Mastercard.
